As anyone who has chatted with me will know, my interest would be the orthodontist - no desire to wear them myself!
Putting the purely kink desires to one side, I'd like to be known for my downsides being outweighed by my results: for example my treatments might take longer, but there never any relapses except if you don't wear your retainers. I might use unpopular appliances like headgear, but the results are always top-notch. I might not allow appliance removal until I was satisfied with the results, but that means the patient gets no regrets later. My favouring neither teeth removal or surgery unless absolutely necessary may make a treatment torturously long, but it's much cheaper... That sort of thing. Think of a permanent state of quid pro quo - a negative for every benefit, and a benefit for every negative.
This would also be known well in advance, so if you came to me, you'd know that even a 'simple' two year treatment would be nearer 3-4, with similar increases as complexity increased. Perhaps also a willingness to work with the patient for certain events (say ideally I'd prescribe headgear for 18 hours a day, but the patient is at college and is willing to take an overall time penalty in favour of night wear only whilst in the dorms), might mean that lenghtier treatments of 8 or 9 years would soon become possible.
