I have a few questions

I'm writing a story about the fall of civilization and thought I'd add in a character or two that have braces. In doing so, I've come across a few queries that I can't find answers to:

1) What happens if braces are left on without any form of maintenance? Will they stay on? Do they fall off?

2) If they stay on, will they adversely effect the teeth in any way?

3) If they fall off, would they leave behind glue or anything? How long before they might start to fall off?

4) Can braces be removed by anyone? Possibly with something as simple as pliers?



I'd appreciate answers to any or all of these questions and thank you in advance!

ad 1) Glue doesn't last forever. Unless the bond breaks by deterioration or by force, the brackes stay on.

ad 2) Depends on the stage of treatment and type of treatment. Often overcorrection is used to allow some relapse. Appliances like expanders, headgear, facemask etc. could adversely affect the bite. Braces itself carry a higher risk for cavities.

ad 3) Yes, glue remains. I guess, the glue would last 5+ years.

ad 4) Yes. The ortho does it with - okay, specialized - pliers.

After re-reading the post, I feel I should elaborate a bit incase it might help with further answers:

After the "fall of civilization" I mentioned, one of the characters the protagonist meets has braces and he asks why they've left them on despite having no one to maintain them, and in response, I had them saying that they met another who had "removed" their's and were left with unsightly glue on each of their teeth and that they'd prefer the braces to that.

According to your reply, Katrinp_99, the other would indeed have glue left on their teeth. Could this be scraped off by non-dental tools? Also, from what you've said, after only 1-2yrs, there's a good chance that the person would still have their's attached, assuming no deterioration or force; and finally, there are no appliances involved so adverse effects should only be limited to cavities, though I'll assume the person has kept up their oral hygiene :P

1) Under ideal, (and impractical), conditions, the bonded brackets could stay there for a really long time. But it is all to easy to loosen one from biting down on something incorrectly or from an impact to the face. And once one has popped off, its neighbors will start coming off, (within a couple of weeks or even days), because the loose bracket and wire is pushing and pulling on the surrounding brackets incorrectly which shears the glue bond. That’s why the ortho needs to fix any loose brackets right away! Any bands could potentially stay on for much longer though, as they can take a lot more abuse than bonded brackets. If your character has full bands he/she could stay a metal mouth for many years if he/she was reasonably careful, ate the proper foods, and was fastidious about oral hygiene.

2) The teeth would move to the point where the archwire had dictated and then stop. If the person kept using headgear, the upper molars would keep moving backwards. That  would end up as a really weird bite as there would be no other forces acting on the other teeth. Same thing with an expander if someone kept cranking it open! Continued use of elastics would also over-push the teeth into less than desirable positions, but since the elastics themselves go bad after a couple of years it would probably not be an issue. Also, stocking up on elastics could be a problem after the fall of Rome! Again, the character would have to be very, very careful about keeping his/her teeth and braces clean or he/she would have major problems with cavities and gum disease.

3) Any glue left behind can easily, (but carefully), be scraped off with whatever is handy. Getting too aggressive with a metal tool could damage the enamel or chip a tooth though. Without any tools at all, the glue would be gone within a couple of weeks from eating and determined picking with a fingernail. Bonded brackets could be gone within first week if the person bit down on something wrong. It is hard to say how long someone could keep bonded brackets in place without regular maintenance from an orthodontist. A mouthful of banded braces could stay put for a long time, if the person was super careful about- you guessed it- hygiene.

4) Bonded brackets can be removed in 10 minutes with a pair of pliers and a mirror but that could also result in damaged enamel and/or chipped teeth if you were not careful. Bands are a bit trickier and usually require the right tools and technique, but someone could do it themselves if they were really determined.

Thanks again for the reply. From the sounds of it, I wasn't too far off. Although if the glue is easily removed, then removal would probably be the more realistic option in such a situation. I might make up another reason why they leave them on. Perhaps they see it as a reminder of "the way life used to be" or something. Thanks again for the help! :)

I thought about your questions too, when I first watched "Lost" ;)

Btw for #1: The glue and bracket is able to hold for way more than just 5 years if you dont bite on rock hard stuff.
But without any maintenance (cleaning) the person would get so much caries that the brackets would fall of anyway

I actualy had a girlfriend (two , realy) who stopped for some time the visits at the orto.
The first thing you´ll notice is the color of the ligatures........... they thend to get brown, and in the second case, the "glue" (they were clear braces) get brown, and the whole braces seem more noticeable.
