For those of you waiting to hear how my appointment went, it had to be rescheduled to Wednesday. I got a phone last Thursday letting me know my orthodontist wouldn’t be in the office the next day. I had the choice to see the new orthodontist in the office or reschedule. Given this would be my last appointment before my surgery, I opted to reschedule so I could see my orthodontist and discuss with her what the plan is after my surgery. The only available day is this Wednesday. So his coming week, I have m pre op appointment and my orthodontic appointment for my regular visit and have my surgical hooks placed. I’m so thankful my doctor filled out my jury duty medical excuse so I won’t have to worry about missing any of these appointments.

I can’t believe it’s only Thursday. It feels like next week should be starting already. I only just got home for the evening from going to dinner with my daughter’s cross country team after their meet, and I left the house at 7:15 this morning. 
Monday, I followed up with my back doctor. Next week, if I have the time, I am due for my next set of in brace and out of brace X-rays. Tuesday, I had my pre-op appointment and was cleared for surgery by my physician. While my ekg hasn’t changed at all, the little computer interpretation thinks there is an issue. My doctor has sent the current ekg along with the 2 prior over to my jaw doctor for his review and the anesthesiologist’s review to make sure they’re both good with it. Just in case, I was miraculously able to get a cardiology new patient appointment for next Wednesday to get an ekg review if needed.
Yesterday I had my orthodontist appointment and had 6 surgical hooks added to my braces. My top jaw has wire ties holding in the archwire in 3 different sections around where my jaw will be cut. It’s the tightest my braces have ever felt, but thankfully was only a little tender overnight. My lower braces got 3 of the hooks along with a new power chain. The best part is I finally get a break from elastics. I’ve been wearing them in one configuration or another for just about 2 years now. The hooks haven’t been too bad. Only the bottom ones have been poking into my lip. The middle one on the bottom wiggles and I can’t stop playing with it. Both the upper and lower set of hooks are spaced weirdly due to the small bends in the archwires to level my front teeth. They look funny.
I’m hoping for a calm day tomorrow. Saturday is my son’s homecoming football game. His team is 7-0 this season and hope to continue the streak. They will be playing a pretty evenly matched team, so it should make for a game.
Next Tuesday is my last appointment with my jaw doctor before my surgery. I’ll get to see my teeth models and get a better idea of how the surgery will go. I have a couple of questions- more out of curiosity than concern. What questions would you ask if you were in my shoes? I would love to hear your thoughts.

The anesthesiologist asked if I had problems with any meds and I explained my resistance t a lot of sedatives relaying how my pulmonologist literally ran out of versed while doing a bronchoscopy a few years ago, so I wasn’t given anything until it was time to put me under. I’m not going to lie, the first day was brutal. I had to wait in the recovery for 9 hours to get a bed in the ICU step down unit. The noise and light made pain management very difficult. I was alternated between fentanyl and morphine to try to help.  I had 5 elastics which all broke in about the first hour and no splint. Most of my jaw was in agony, my face and palate were numb and everything was in a different location than I was used to. I couldn’t swallow. When I finally got to my room, I was given a PrimaFit external catheter. It’s hooked up to constant suction that slurps away urine as you are peeing- if you’re able to force yourself to pee while not on a toilet. It took me hours and I thought I was going to explode. I had to bear down so hard, I do not recommend unless you have incontinence and are bed bound. I would have happily kept the foley that was placed in the OR.
After that my nurse tried to help me start swallowing. With both jaws moved forward and no feeling on my palate, my tongue didn’t know where to go. My mouth was technically made bigger but felt smaller. I coughed and choked while worshipping my suction wand. Slowly a few drops at a time I was able to get a couple ounces of apple juice down after not eating for 27 hours, even though I wasn’t really hungry. I was discharged Wednesday afternoon only after my doctor was able to confirm that all the prescriptions he ordered were able to be filled. I had to go by his office on my way home to have my elastics put back in and a CT scan taken. You can see in the CT how swollen my face is. https://dereferer.me/?https://imgur.com/a/uVu6cSt The surgery was different than I had originally been told and not everything could be done because there was no way to secure all of it. I didn’t find out the changes until the Tuesday before surgery. My upper and lower jaws were both moved 6mm forward, and my chin was moved an additional 6mm forward. The top jaw was slanted slightly to even out my bite and the bottom jaw was tilted slightly to even my midline. No jaw widening could be done because of the needed forward movement. My doctor phrased it to the effect of with this procedure no widening could be done, though he did acknowledge that I still don’t have enough room in my mouth for my tongue. I don’t know what to make of the conversation, especially while under the influence of my pain meds. I now have eight elastics that I have to replace on my own when they break and it’s painful. I can’t open my mouth enough to show them to you at this time.
Yesterday I developed a horrible allergic reaction on my nose where they used liquid adhesive to help hold my intubation tube in place. I’ve had to text updates several times a day to my surgeon to make sure it isn’t getting worse. When I spoke to him this evening (if you want to call it speaking, he asked how my elastics were doing. I told him still breaking. I begged him before surgery to use anything else afterwards because I have watched videos of people have to change theirs after surgery and it looked excruciating, and Im here to tell you those videos don’t lie. He seems to think I’m opening my mouth and snapping them, but that’s not the case. He mentioned that if that was what is causing them to break, wiring my jaw would be worse and could loosen my hardware from my pulling against the tension. We’ll see what he decides when I go back on Tuesday. I’m very grateful that I can only feel part of the irritation, otherwise I would have scratched my face off by now. I’ve had to take 50mg of Benadryl every 6 hours to try to get it to calm down. In addition to that, I’m on liquid Vicodin and liquid antibiotics. Before surgery, I walked the children’s over the counter section and picked up the liquid form of several meds like Benadryl (just in case, and very good call), my daily Zyrtec, but also peptobismol and ibuprofen (I think I have to drink half the bottle to get an adult dose).
My daily diet is a choice of flavors of different flavored nutritional shakes. I have cafe au lait, pumpkin spice, cinnamon roll, a couple of chocolates, and mixed berry or orange clear boost. I also have some Trader Joe’s power of 7 purple and red juices to get some fruits and veggie nutrients. I have the usual gatorades and electrolyte drinks too. Lastly, I have chicken, turkey, and beef broth when I want something savory. I can’t remember having to pee so much in my life. I don’t know how I’m going to keep myself from floating away.
Tonight a good friend of mine from Panama brought a home cooked meal for my family. It smelled so good, but I wasn’t in the least bit tempted to try to eat it. I’m blaming my meds for that. My son loved her cooking. It’s the first time he ever tried plantains. I told him I like them with ketchup, but he might like Cane’s sauce better. They were a hit! She promised to have us over for authentic Panamanian food once I can eat again. It’s only been 28 years since I’ve eaten in Panama, what’s a couple more months, right?

And again I am impressed by your strength to undergo such a large operation, that may leed to all the results you wish.

And are you saying that you might need another one to widen your jaw?

You write that no widening of your jaws could be done, so you might need another operation.

Maybe you could avoid those operations with expanders?

Posting a little late. I had my first follow up appointment on Tuesday. My surgeon is very happy with the results and how well I am healing. I am not as swollen or bruised as he expected. I let him know how much I have been struggling with my elastics and the issue with frequent breakage, in part because I am grinding my teeth now that my bite is no longer aligned because my jaws are now anatomically aligned if that makes sense. Also because I’m pushing my tongue between my teeth with my tongue thrust. He changed the pattern slightly and posed perhaps trying the latex to see if they bother me or not. He also told me I was looking at another 12 months in braces once I am cleared to go back to my orthodontist. All I heard when he told me that was another 12 months of elastic hell. I’m not going to lie, I love my braces. They’ve been amazing, but elastics are the devil. My orthodontist had said 8 months of braces after surgery. While I’d like to believe her, by her account from when I started treatment with her, I should already have them off. My guess is my surgeon is probably more accurate in the time estimation.
We really didn’t discuss anything else.
I continued to struggle with the elastics not providing support or stability for the last two days. Last night was miserable. Somewhere around 2 am I woke with most of the elastics broken and my jaw not closing completely on the left side as if there were something pinched in the joint. I evened out the elastics and tried my best to sleep. By the time I woke up my jaw would close but still a lot of pain. I texted my doctor asking if I could try the latex elastics wondering if it was a brand thing or actual latex allergy. He told me to come into the office.
Once I was called back I explained in detail the previous night’s experience. He installed the latex elastics and for the first time I felt relief in my jaw. He wanted to do another CT scan to check the discs. By the time I came back, my cheeks were tingling and starting to itch and my saliva was causing a tickle in my throat. I wanted to cry. He took out the elastics and replaced them with the non latex ones. He increased the amount and changed the pattern again.https://dereferer.me/?https://imgur.com/a/rBiMYzW It wasn’t as supportive as the latex version, but much better than what I had previously. My CT scan was ok which was a relief. Ironically while he was putting my elastics back he saw my tongue come forward and scolded me. I told him my tongue tamers were useless and he had already noticed. He said he kept poking himself with them throughout my surgery. He was surprised to see my tongue not be bothered by them in the least, but also pointing out I need to really work on it. I reminded him I was in the process when my treatment plan changed to focus on surgery, then caught myself wondering if this will add more time to the 12 months longer of wearing braces- which in the absence of elastics wouldn’t be horrible. I found it a good time to ask about the issue with my jaw width. Essentially it’s the way it will be. My tongue will be too big and my jaws to narrow because my lower jaw isn’t wide enough to accommodate any maxillary expansion. There was no way to widen my lower jaw with the amount of expansion that I needed. He then pulled up my before surgery CT and compared it with today’s. To say I was blown away would be an understatement. I’m curious to know if my airway issues have been lifelong or something that has recently developed. I say this because having been a competitive swimmer through college and my with my time in the military- how much better could I have possibly been if this has been going on my whole life and was addressed sooner? I was exhausted once I got home as I’m still recovering from quite a bit of blood loss in addition to little sleep last night. I took a nap and of course two elastics broke. In order to get the broken ones out, I had to move two intact ones. I got those back on, but can’t for the life of me get the broken ones replaced without excruciating pain. I tried again this evening to no avail. I don’t know if I should suck this up until Tuesday, or bother him again tomorrow. I’m so over it.
For those of you who may be wondering, I started wearing my back brace again last Friday. I’ve noticed tremendous improvement in my airway so I wasn’t so concerned about my back brace causing any respiratory complications like I was after my ankle surgery. I’m getting between 13 and 16 hours of wear daily. I’m in the limbo stage where I can feel the discomfort of my spine moving when wearing it and need a break coupled with my back hurting if I’m not wearing it. The mornings are the hardest right now waking up with back pain, and I haven’t been diligent with putting it back on immediately once my back stops hurting. I’ll get there again soon though. Tomorrow I have some driving to do with my husband being out of town so I’ll definitely be wearing it then.

The elastic things suck. I hated my elastics as well, and I had the regular latex ones. But they too would snap every single night. I don’t know how much latex I swallowed during my treatment.

It is always interesting to read about your development.

I do not know anything about the elastics.

Concerning your tongue problems maybe a tongue crip might help better than those tamers?

These last few days have been absolutely miserable. While the parts of my jaw that were operated on are doing well, my TMJ has been excruciating. My elastics do not help in any way shape or form. I have not been able to replicate the elastic pattern my surgeon used to get the most stability. Even taking my newly prescribed muscle relaxer did nothing. It got so bad that Thursday night I ended up having to take Vicodin (the liquid version tastes horrible) again after not needing it for post op pain for several days. I texted my surgeon Friday morning that I was at wits end.
I had my routine post op appointment today. Everything continues to improve except the jaw pain. I know this sounds funny, but my eustacian tubes have never been better. For the first time in my life, my ears pop normally when swallowing, and I’ve had no problems with any fullness or fluid in my ears which prior to now had been a pretty regular occurrence. Hurray for the small things. My doctor examined my jaw thoroughly and is still very pleased with the results as am I. When he got to my jaw joints, I about jumped out of my chair, audibly whimpered, and started to cry when he pressed on the joint. Most of you have been following me on my journey for over a year now and can attest that this is the first time with everything I’ve been through that I have cried in pain. It turns out the pain is from a severe muscle spasm. I was told I would most likely need Botox injections to get complete relief, but that can’t be done for 6-12 more weeks. In the meantime, I was offered to have my jaw wired to see if that helps. I did not hesitate to agree. The process went quickly. It’s only been 2 1/2 hours but I have already noticed some improvement. I’m hoping this will provide substantial relief. I can go back at any time if I want them removed, otherwise he will re-evaluate everything in a week at my next post-op appointment.
That’s pretty much everything in a nutshell. I’ll let you know if I end up going back before my next scheduled appointment, or if this is the solution for my horrific pain.

Why does this keep happening? One step forwards, two steps back, two steps forwards, one step back. This whole thing is so up and down.

And that first picture is brutal. The fact that we can see the muscle spasms is actually insane.

Well, it’s been 6 days since my jaw was wired shut. It’s definitely been an adjustment, but I’m at a point where I don’t really notice unless I try to talk or when I’m eating (drinking). My left TMJ is not nearly as painful as it has been. Unfortunately the swelling in the side of my face worsened over the last 2 days. https://dereferer.me/?https://imgur.com/a/fXxC3HZ I reached out to my surgeon yesterday early afternoon and sent him a picture. He decided to put me back on antibiotics as he wasn’t sure if it is an infection or a muscle spasm. Four hours after starting the antibiotics, the swelling was even larger. At that point I had the option of going to the ED who would have had my surgeon come in and evaluate me or go to his office at 8:30 on a Saturday night. I obviously opted for the doctor’s office. I took my temp before leaving and while I didn’t have a fever earlier in the day, it was up to 100.3F. Once I got there, he did a thorough assessment. He was on the fence as to weather it’s a spasm of the masseter muscle, a salivary blockage, or an infection. I had thought earlier it could be a salivary gland blockage and tried drinking sour liquids, but I didn’t experience any of the usual sensations of when your glands try to create extra saliva so I don’t think that’s the issue. The possibility of infection in that area is very unlikely because none of it was exposed during my surgery, but remotely possible. That being said, he still wants me to continue with the antibiotics. He noticed my left TMJ wasn’t as bad as it was on Tuesday but really thinks it’s another muscle spasm, even though my pain level is nowhere near where it had been earlier in the week. It’s not to say that the exam wasn’t painful, but I wasn’t crying this time. He did say I could start taking ibuprofen which has really helped with the discomfort along with heat and continuing the muscle relaxer 3 times a day. I suspect it’s a mix of infection (based on my overall feeling like crap and fever) combined with a muscle spasm (maybe triggered from the infection?) Fortunately today it has not gotten any bigger, so I’ll take that as a good sign regardless of the cause. I go back on Tuesday for my regular post op visit unless things take a turn for the worse. Either way, I’ll let you know.
